ARTICLE XV Insurance Benefits
90. 1. Group Life Insurance. Group life insurance is available to all employees effective upon the first (1st) day of the month following the date of employment at which time no medical examination is required.
This insurance is contributory. The employee will contribute fifteen (15) cents per thousand (1,000) per month, the balance being paid by the Company.
Dependent life insurance is also available.
91. Group Accidental Death and Dismemberment Insurance equal to the amount of Group Life Insurance carried by the employee, to a maximum of $10,000, is available to all employees who carry Group Life Insurance at no cost to the employee.
92. Any employee who does not wish to subscribe to the life insurance when eligible shall be subject to a medical examination if, at a later date, she decides to participate.
93. 2. Employees are eligible for the following benefits, the terms which are set forth in the respective Summary Plan Description booklet:
