86. 1. The Company will pay disability benefit allowance to any eligible employee who, as the result of injury or sickness not arising out of and in the course of his employment, is unable to perform his work or other work which may be offered to him. The terms of this disability benefit are as follows:
Short Term Disability Benefits
Under the ISP Short Term Disability Program (STD), you receive pay for a certain number of weeks that you are determined to be disabled (proof is required) and are under the care of a doctor, for up to 26 weeks in a 52-week period as shown in the following schedule. Short Term Disability is a Company Paid Benefit.
Benefits up to the maximum are payable during one continuous or intermitted period of total disability. An employee is limited to 26 weeks of disability in a fifty-two week period. Per attached schedule.

Note for clarification: The first three days of sickness (no pay) is per occurrence. Exception to the vacation policy to allow employees to use their available vacation to receive pay on days 2 and 3 of any multi- day absence. Periods of disability due to sickness will start on the 4th day, per occurrence. To receive benefits, you must be under a doctor s care and have medical proof of your disability Doctor s excuse is required on the first paid day and all subsequent paid days of illness/ sickness.
If you exhaust your STD benefit in a nonconsecutive 26 week period, you were approved for and are medically qualified to continue STD (based on your will receive weekly payments of $800, for a maximum of four (4) additional weeks. At the end of this period, the employee may continue to pay insurance premiums (Medical, Dental, Vision, Prescription, Life, and LTD) at the active employee cost share until you have reached the 26 weeks to qualify for LTD and/or PTD.
For 8-hour, Monday through Friday workers ONLY:
Employees w through Friday working hours. Time off allowed will be a maximum of 4 hours
per visit and a maximum or 4 occurrences per year. Time off required can be made up at straight time pay provided APPROVED IN ADVANCE by the Department Manager.
87. 2.
Kentucky shall receive eighty five percent (85%) makeup pay for the first thirteen (13) weeks and one hundred percent (100%) for the next thirty nine (39) weeks, not to exceed a total of fifty two (52) weeks.
88. 3. No payment shall be made unless the employee complies with instructions of tment regarding his medical care.
89. 4. Employees who are disabled due to injury or sickness arising out of and in the course of their employment shall retain and accumulate seniority until such time they are totally and permanently disabled.
